We may use and disclose PHI to obtain payment for services rendered. Refine You is a cash-only practice and does not bill insurance, Medicare, or Medicaid.

We may use PHI for practice operations, quality assessment, training, licensing, and compliance activities.

d. Business Associates

We may disclose PHI to trusted third parties (e.g., EHR systems, billing or payment processors) who are required to safeguard your information.

We are required to notify you following a breach of unsecured PHI.
